With the evolving healthcare landscape, changes in reimbursement models for services and the projected physician shortage by 2025, it is necessary to consider ways in which we can leverage the expertise of our healthcare team members. We have done this successfully through our primary care medical home pharmacy practice model. It has proven to be effective within U-M Health System (UMHS) and provider satisfaction is overwhelmingly supportive.

Our programs span both within and outside of UMHS. We have opportunities for partnerships through many programs and we can work with your practices and organizations to develop models or partner with one of our existing models. These include:

  • Expansion of pharmacy practice into specialty clinics
  • Collaboration between the UMHS patient centered medical home and community pharmacies
  • Integration of telehealth as an approach to patient care
  • Implementation of employer-based, comprehensive medication review program
  • Establish and promote safe, appropriate, and cost-effective therapy


Our current partners include:

Patient Centered Medical Home Pharmacist Practice Model – Eleven pharmacists are embedded into the UMHS primary care clinics and provide disease management and comprehensive medication review services. As an organization, we have seen significant improvement in diabetes and hypertension quality metrics as a result of this program.

Specialty Clinics – Similar to PCMH practice model, we are expanding into specialty clinics, such as Chronic Kidney Disease, Psychiatry, and Anticoagulation/Cardiology to further coordinate and collaborate with our medical neighborhood.

Community Partnerships

  • Meijer Community Blood Pressure CentersCollaboration with community pharmacies to provide coordinated hypertension monitoring services to UMHS patients. Currently we have partnered with Meijer Pharmacy to provide hypertension management services to UM patients in the local community.

POM Accountable Care Organization – We are working with our ACO partners to assist them in developing a similar PCMH Pharmacist Practice Model in their organizations.
